SUBJECT: BILLING AND PAYMENT

       Starting the month of October 2002, EVISNet staff will no longer visit your residence or offices to collect your monthly Internet subscription. This will enable EVISNet to improve its efficiency in collection and thereby work on other ways to improve its services.

       Because of the change in the procedure of collection, all subscribers are REQUIRED to pay their monthly subscription at the following payment center and schedule:

1. NEDA Regional Office (Monday to Friday)
   Ground Floor NEDA Region VIII Building
   Government Center, Palo, Leyte
   Tel.#: 323-3110 - c/o Gemma Militante
   
   Note:  AN OFFICIAL RECEIPT WILL be issued and will be attached on bill for the
   succeeding month
   
   
2. EVISNet Office
   Ground Floor NEDA Region VIII Building
   Government Center, Palo, Leyte
   Tel.#: 323-7214
   
   Note:  In case the cashier is not available upon payment, kindly proceed
              at EVISNet office
   

• Failure to pay the amount due within fifteen (15) days from the date of billing will be added to the next billing month and failure to pay within another fifteen days will mean TEMPORARY DEACTIVATION of your SUBSCRIBER DIAL-UP Account.
  
  
• An account shall only be reactivated after fees and dues are SETTLED.
